This model can help in improving the health systems’ performance by promoting the right balance between optimal treatment outcomes and total spending, especially crucial for chronic diseases.

The key element of VBHC is to create a value chain where long-term outcomes (clinical, quality of care & spending) for all the stakeholders can be improved while containing the total costs.
